Notes Required when
- On return to school after child has been off sick
- Child has to stay in at playtime
- Child has to be excused from some school activity
- Child has to leave school early or is being collected by some adult not known to the teacher
- Child is unable to do homework
- Child is not wearing school uniform
- Prior to family holiday during school year
